G-6-PD缺乏症在新生儿高胆红素血症的影响及其特点

摘    要:目的探讨红细胞葡萄糖-6-磷酸脱氢酶(G-6-PD)缺乏症在广西新生儿高胆红素血症的影响及其特点。方法对2869例广西籍新生儿高胆红素血症患者进行G-6-PD活性检测,对确诊为G6PD缺乏症608例患者的临床资料进行分析。结果男女婴儿G-6-PD活性比较,男婴G-6-PD活性低于女婴,(P=0.002〈0.01),差异有显著性;男女婴血清总胆红素、血红蛋白、黄疸出现时间、黄疸消退时间比较均无明显差异(P〉0.05)。G-6-PD活性与血清总胆红素、血红蛋白、黄疸出现时间、黄疸消退时间,差异无统计学意义(P〉0.05);血清总胆红素与血红蛋白,呈线性相关,有统计学意义(P〈0.05)。结论G-6-PD缺乏症的新生儿,黄疸出现时间早、进展快,胆红素脑病发生率高。男女婴G-6-PD活性的差异有显著性,男婴明显低于女婴;但男女婴血清总胆红素、血红蛋白、黄疸出现时间、黄疸消退时间比较均无明显差异。并不是G-6-PD活性越低,引起的黄疸程度越重、贫血越重、黄疸出现时间越早、黄疸消退时间越慢。但G-6-PD缺乏症引起的血清总胆红素越高、血红蛋白越低。

Abstract:Objective To explore the red blood cell glucose-6-phosphate dehydrogenase(G-6-PD) deficiency in neonatal hyperbilirubinemia in Guangxi and its characteristics.Methods G-6-PD activity was detected on 2869 cases in Guangxi nationality patients with neonatal hyperbilirubinemia,Analysis of 608 cases of clinical information on patients diagnosed as G6PD deficiency.Results G-6-PD activity of male and female,male the G-6-PD activity lower than the female(P =0.002<0.01),significant difference.male and female serum total bilirubin,hemoglobin,the time jaundice appear,the time jaundice dissipated,there was no significant difference(P > 0.05).G-6-PD activity and serum total bilirubin,hemoglobin,the time jaundice appear,the time jaundice dissipated,there was no significant difference(P >0.05);Serum total bilirubin and hemoglobin,showed a linear correlation,was statistically significant(P <0.05).Conclusion G-6-PD deficiency in neonatal,jaundice appears earlier,rapid progression,high incidence of bilirubin encephalopathy.G-6-PD activity of male and female,male the G-6-PD activity lower than the female;but male and female serum total bilirubin,hemoglobin,the time jaundice appear,the time jaundice dissipated,there was no significant difference.Not G-6-PD activity,the lower the degree of jaundice caused by the heavier,the more severe anemia,jaundice occurs sooner,the jaundice dissipated the slower time.However,G-6-PD deficient serum total bilirubin caused by the higher,the lower the hemoglobin.
