慢性肺源性心脏病病人血流动力学研究及硝苯吡啶对其影响

摘    要:10例慢性肺心病人行Swan-Ganz导管检查,同时行血气分析以及肺功能、超声心动图等检查,并观察了舌下含服硝苯吡啶对血流动力学等参数的影响。结果显示肺动脉压(PAP)等血流动力学参数与PaO_2等参数呈显著相关,PaO_2等可能是预计PAP的有用参数。持续吸氧,纠正低氧血症,有利于防治肺动脉高压。舌下含服硝苯吡啶后PaO_2显著降低,肺内生理分流增加,体循环血压显著下降,但PAP未降低。故不宜长期使用硝苯吡啶舌下含服治疗肺心病和肺动脉高压。

Abstract:wan-Ganz catherization blood gas analysis,pulmonary function tests and two-dimensional echocardiography were perfoimed in 10 patients with pulmonary heart disease and the effects of nifedipine were then observed.Mean pulmonary artery pressure (mPAP)correlated significantly to PaO_2.PaO_2 may be a useful parameter for predicting mPAP. Continuous oxygen breathing can correct hypoxemia,prevent and alleviate pulmonary artery hypertension(PAH).After using nifedipine,PaO_2 and blood pressure of systemic circulation decreased significantly, while pulmonary physiological shunt increased and pulmonary artery pressure did not decrease. As a consequence,the use of nifedipine to treat pulmonary heart disease and PAH is not advocated.
