高海拔地区妇产科危重症并发多器官功能失常综合征的诊断标准和综合治疗的临床研究

目的：探讨高海拔（２２６０ｍ）以上地区妇产科危重症发展过程中并发多器官功能失常综合征（ＭＯＤＳ）的规律，以期早期诊断和及时治疗，达到提高抢救成功率、降低病死率的目的。方法：参考国内外ＭＯＤＳ的诊断标准，结合高海拔地区高寒低氧的特定环境和条件，以及妇产科重症疾病的特点，总结近１０年来收治的９８例ＭＯＤＳ抢救治疗体会，制定出适合高海拔地区条件的早、中、晚期ＭＯＤＳ诊断标准，并总结出中西医结合的有效综合抢救措施。结果：按诊断标准发现早期ＭＯＤＳ患者３５例，中期２８例，晚期３５例，其病死率分别为０（０／３５），１７．９％（５／２８），５１．４％（１８／３５），明显降低了病死率。结论：在高海拔地区妇产科危重病易发生ＭＯＤＳ，因此对妇产科危重症用本研究的诊断及程度评分标准能早期对ＭＯＤＳ做出诊断，及时应用中西医结合等综合治疗措施，对降低ＭＯＤＳ的病死率有重要意义。

DIAGNOSIS AND TREATMENT OF MULTIPLE ORGANS DYSFUNCTION SYNDROME(MODS) ASSOCIATED WITH THE CRISIS OF GYNECOLOGY AND OBSTETRICS AT HIGH ALTITUDE

Objective:To explore relationship between MODS and crisis of gynecology and obstetrics for early diagnosis and treatment at high altitude (2 260m). Methods:After the previous criterion of MODS was revewed,there are few data avaitable regarding the diagnosis of MODS at high altitude.Therefore,98 cases with MODS,who administed in our hospital during 10 years,were evaluated,specially diagnosis and treatment protocol were made out through combining general criterion with high altitude hypoxic environment and therapy of westen medicine with chinese tranditional medicine.Results:The number and mortality of the patients with early, middle and advance MODS were 35,26 and 35 cases and 0,17 9% and 51 4% seperatly on the protocol.The mortality of these cases was remarkably decrease compared with previous data.Conclusions:The protocol are useful in diagnosis and treatment of MODS associated with crisis of gynecology and obstetrics at high altitude.
