乌司他丁对心肺复苏患者脑复苏的作用及其预后的研究

目的 观察乌司他丁对心肺复苏患者脑复苏的作用及其预后的影响.方法 随机选择2006年10月至2008年12月45例恢复自主循环的心肺复苏患者作为研究对象,分成两组:T组在自主循环恢复后除采用常规的支持治疗外加用乌司他丁,C组采用常规的支持治疗.所有患者均在自主循环恢复后即时、复苏后1、3、7 d分别进行格拉斯哥评分(GCS),同时观察患者的住院时间、机械通气时间、病死率、多脏器损害发生率等指标.结果 自主循环恢复即刻、1 d两组的(GCS)差异无统计学意义(P>0.05),但复苏后3、7 d的(GCS)差异明显(P<0.05).T组的住院时间、机械通气时间、病死率、多脏器损害发生率均较C组明显的减少或降低(P<0.05).结论 乌司他丁有利于心肺复苏患者的脑复苏,缩短存活患者的住院时间、保护重要脏器的功能,降低心肺复苏患者病死率,有助于改善心肺复苏患者的预后.

Study on Effects of Cerebral Resuscitation and Prognosis of Ulinastatin in Cardiopulmonary Resuscitation Patients

Objective To investigate the efect of cerebral resuscitation and prognosis of ulinastatin in cardiopulmonary resuscitation patients.Methods A prospective randomized controlled study was performed involving 45 restoration of spontaneous circulation(ROSC) patients admitted to emergency department from October 2006 to December 2008.On admission,patients were randomly assigned to two groups:group T using routine supportive treatment and ulinastatin,group C using routine supportive treatment.The questionnaires of glasgow coma scale(GCS),length of stay in the hospital for survival,length of ventilatory support,the mortality,the incidence of multiple organ dysfunction syndrom were compared between two groups.Results GCS at 0,24 hour after ROSC had no significantly diference in the two groups,but significant changes were found at 3 day and 7 day(P<0.05).Mortality is lower,length of stay in the hospital for survival,length of ventilatory support,the mortality,the incidence of multiple organ dysfunction syndrom were less or lower in group T than those in group C(P<0.05).Conclusion Ulinastatin is helpful to improves cerebral resuscitation and Prognosis,reduce motality and shorten the length of stay in the hospital for survival,protect vital organ function in patients with cardiopulmonary resuscitation.
