PAI-1、Fg与2型糖尿病患者颈动脉硬化的研究

目的分析循环血中纤溶酶原激活物抑制物-1(PAI-1)、纤维蛋白原(Fg)在2型糖尿病人群及非糖尿病人群中的差异。探讨PAI-1、Fg与2型糖尿病患者颈动脉硬化的相关性。方法对158例心内科住院患者及42名健康体检者检查血糖、空腹胰岛素、血脂、尿酸(UA)、血常规、凝血三项、PAI-1、Fg,测量血压、腰围(WC)、身高和体重,并计算体重指数(BMI);通过颈动脉超声测量颈动脉内膜中层厚度(IMT)。分析PAI-1、Fg与2型糖尿病患者IMT的相关性。结果与正常对照组人群相比,2型糖尿病组患者PAI-1、Fg水平明显升高(P<0.05),2型糖尿病合并颈动脉硬化组患者PAI-1、Fg水平均显著高于2型糖尿病无颈动脉硬化组患者及正常对照组人群(P<0.05)。多元回归分析显示,2型糖尿病患者的血浆PAI-1、Fg水平与颈动脉IMT独立相关,提示体内PAI-1、Fg水平的高低与2型糖尿病患者颈动脉硬化程度密切相关。结论 PAI-1、Fg水平升高可能是2型糖尿病合并大血管病变的危险因素。

Relationship between Carotid Atherosclerosis and PAI-1,Fibrinogen in Type 2 Diabetes Mellitus

Objective To investigate the correlation of inflammatory markers plasminogen activator inhibitor-1(PAI-1) and fibrinogen(Fg) with carotid atherosclerosis in patients with type 2 diabetes mellitus(T2DM).Methods Two hundreds subjects were enrolled,including 158 in-patients and 42 health controls.Systolic blood pressure(SBP),diastolic pressure(DBP),waist circumference(WC),body mass index(BMI),plasma fasting blood glucose(FPG),fasting insulin,lipids,uric acid(UA),and the blood levels of PAI-1 and Fg were measured in all subjects.Correlation of PAI-1 and Fg levels in T2DM patients with carotid atherosclerosis was analyzed.Results The blood levels of PAI-1 and Fg in patients with T2DM were higher than those in health controls(P<0.05).The PAI-1 and Fg levels in T2DM patients with carotid atherosclerosis were significantly higher than those in T2DM patients without carotid atherosclerosis(P<0.05).The stepwise regression analysis showed that the blood levels of PAI-1 and Fg were independently associated with the carotid IMT.Conclusion T2DM with carotid atherosclerosis may be correlated with the high levels of PAI-1 and Fg,which may be risk factors of T2DM with macroangiopathy.
