肾移植术后严重肺部感染24例临床分析

目的 了解肾移植术后严重肺部感染特点.方法 对24例肾移植后发生肺部感染者的临床资料进行回顾性分析.结果 32例次痰培养提示病原微生物生长,10例肺泡灌洗中7例检出病原体;治愈15例(63%),死亡9例(37%).其中ARDS 13例,死亡6例(46%).在双肺影像学检查提示呈磨玻璃状者的9例中治愈7例(78%),死亡2例(22%).结论 肾移植术后严重肺部感染的病原体较为复杂,除细菌、结核感染外,真菌、肺孢子菌、病毒也较为常见,临床上争取早诊断、早治疗对降低病死率有重要意义.

Clinical observation of 24 cases pulmonary infection after kidney transplantation

Objective To observe the clinical feature of pulmonary infection after renal transplantation.Methods The clinical data of 24 patients with the pulmonary infection after renal Transplantation were retrospectively analyzed.Results Microorganisms were detected in 32 sputum cultures out of 24 kidney transplantation patients with the pulmonary infection.Through positive res- cue treatment,15cases(63%)was cured and 9 died.6 cases(46%)died in 13 ARDSs.Among imaging diagnosis of 9 cases showed extensive regions of ground-glass opacity,7cases(78%)was cured and 2(22%)died.Conclusion The Pathogen of pulmonary in- fection after renal transplantation is complicated,The infection of mycetes pneumocystis and virus seen frequently except bacterial and tuberculosis.The diagnosis and active treat in early stage contributes to decrease the case fatality.
