急性脑梗死患者血清游离脂肪酸和铁蛋白水平变化及其临床意义

摘    要:目的探讨急性脑梗死患者血清游离脂肪酸和铁蛋白水平变化及其临床意义。方法选择我院神经内科2011年1月—2014年2月收治的急性脑梗死患者95例作为病例组,同期在我院体检健康者50例作为对照组。根据梗死灶直径将病例组患者分为大面积组41例(梗死灶直径5 cm)和小面积组54例(梗死灶直径≤5 cm)。检测病例组患者入院次日(急性期)、出院当日(恢复期),对照组受检者体检当日血清游离脂肪酸和铁蛋白水平。结果小面积组、大面积组血清游离脂肪酸和铁蛋白水平均高于对照组,大面积组血清游离脂肪酸和铁蛋白水平均高于小面积组(P0.05)。病例组患者急性期血清游离脂肪酸和铁蛋白水平均高于恢复期(P0.05)。直线相关分析结果显示,病例组患者血清游离脂肪酸水平与血清铁蛋白水平呈正相关(r=0.509,P0.01)。结论急性脑梗死患者血清游离脂肪酸和铁蛋白水平升高,且两者与脑梗死灶大小、病情严重程度有关,两者可能共同参与了急性脑梗死的发生、发展;临床上应重视急性脑梗死患者血清游离脂肪酸和铁蛋白水平的监测。

Abstract:Objective To investigate the changes and clinical significance of serum levels of free fatty acid( FFA)and ferritin( SF) in patients with acute cerebral infarction( ACI). Methods A total of 95 ACI patients were chosen as case group from January 2011 to February 2014 in our hospital,and then they were divided into groups A( n = 41,infarcts diameter〉 5 cm) and B( n = 54,infarcts diameter≤5 cm),according to infarcts diameter; at the same time,50 healthy cases in our hospital were chosen as control group. The serum levels of FFA and SF of ACI patients were detected on the second day after admission( acute stage) and on the day of discharge( restoration stage); those of control group were detected in the morning on the day of physical examination. Results The serum levels of FFA and SF of A group and B group were higher than those of control group,respectively,and those of A group were higher than those of B group( P〈0. 05). The serum levels of FFA and SF of the 95 ACI patients at acute stage were higher than those at restoration stage( P〈0. 05). Linear correlation analysis showed that,serum FFA level was positively correlated with serum SF( r = 0. 509,P〈0. 01). Conclusion The serum levels of FFA and SF of ACI patients are higher,and both FFA and SF play an important role in the occurrence and development of ACI.
