我院静脉药物配置中心差错分析及防范措施研究

目的探讨防范差错重复发生的有效措施,保证临床用药的安全。方法对静脉药物配置中心工作中出现的各类差错进行分析,针对性地提出防范措施。结果各种类型差错中,排药差错、医嘱审方差错、配置差错为主要因素;复核失误为次要因素,退药差错、打包归箱差错等为一般因素。其差错可以通过多种措施进行防范。结论通过制定严格的操作和配置规范,并加强管理等防范措施,能将差错降低,保障患者输液安全。

Analysis of errors in pharmacy intravenous admixture service of our hospital andpreventive measures

Objective To discuss the effective measures to reduce errors for ensuring the drug safety for clini- cal medication. Methods Different types of errors that appeared in the work of pharmacy intravenous admixture serv- ice were analyzed and the targeted preventive measures according to the reasons were adopted. Results In all the types of errors, the ranked drug errors, doctor trial square errors, configuration error were the main factors ; review of mistakes were the secondary factors, drug withdrawal errors, mistakes and other packing boxes were the general normalization factors. The errors can be prevented through a variety of measures. Conclusion By enacting strict operations and con- figuration specifications, strengthening management and other preventive measures, can reduce errors and protect pa- tients transfusion safety.
