| 摘 要： | 目的 通过对肝细胞癌切除术后无瘤生存影响因素分析，探讨肝细胞癌根治性切除术的标准的建立。方法 运用Ｃｏｘ比例风险生存分析模型对１，４５７例肝细胞癌切除术患者的临床病理因素进行影响因素分析，无瘤生存率采用Ｋａｐｌａｎ－Ｍｅｉｅｒ法计算。

| Abstract: | Objective To explore the establishment of a criterion for the curative hepatectomy for hepatocellular carcinoma (HCC). Methods Retrospective study was used to analyse 1,725 cases with HCC after hepatectomy.The following-up rate was 84.5%.The significant factors were analyzed by the Cox's proportional hazard model,and the disease-free survival was analyzed by Kaplan-Meier estimation. Results The results showed that the most significant prognostic factors to disease-free survival were as following:preoperative portal thrombus,surgical margin,tumor thrombi on surgical margin,gross type of tumor,tumor capsule,daughter nodules,vascular invasion and postoperative alpha-fetal protein. Conclusion The establishment of a criteria of curative hepatectomy for HCC are heplful for appraising disease-free survival after hepatectomy and choosing the suitable palliative treatments pre-and post-operatively. |
