Orem自理理论在脑出血患者术后护理中的运用

目的:降低脑出血患者术后的致残率,提高生活自理能力与生存质量。方法:将73例脑出血微创血肿碎吸术后患者分成观察组和对照组,观察组在整体护理基础上应用Orem自理理论评估患者及家属的自理能力,在各阶段采用不同的护理系统及健康教育,使患者及家属共同参与护理活动,对照组仅实施整体护理。两组康复前后进行临床神经功能缺损评分、改良Fugl-meyer评分和Bathel指数评分。结果:观察组临床神经功能缺损评分、改良Fugl-meyer和Bathel指数评分明显优于对照组(P<0.05)。结论:将Orem自理理论用于脑出血患者术后护理,可降低致残率,使患者最大限度地从身心残障中恢复,重返社会,提高生存质量。

Department of neurology, the First People’s Hospital of Chenzhou, Chenzhou 423000, China. 【Abstract】Objective To cut down the disability rate of post-operative patients with cerebral hemorrhage and to improve the self-care ability and living quality. Methods Seventy-three patients with cerebral hemorrhage who treated with Minimally Traumatic Puncture Drainage (MTPD) were divided into intervention group(n=36) and control group(n=37). The self-care ability of both patients and their relatives was evaluated by Orem self-care theory, who were taken difference nursing systems and health educations according to the recover stages and were made to co-participate the nursing activity. Clinic Nervous Function Defect Assessment and modified Fugl-meyer and Bathel index Assessment were invited to patients’ recover. Results The index assessment points of modified Fugl-meyer and Bathel index was higher in the intervention group than that in the control group significant (p<0.05) Conclusion The application of Orem self-care theory in the post-operative patients’ nursing plan can degrade the disability rate which benefits the physical and mental recovery as well as helps themself return to the society and enjoys a quality living.
