2型糖尿病血小板-单核细胞聚集体水平及其与CD40、CD40L的关系研究

目的:观察2型糖尿病外周血的血小板-单核细胞聚集体、CD62p及其与CD40、CD40L之间关系变化,并探讨2型糖尿病微血管特异病变的发病机制和临床识别和预测的指标。方法:流式细胞术检测2型糖尿病病人血小板-单核细胞聚集体、CD62P以及CD40、CD40L。结果:有微血管病变的糖尿病组的血小板-单核细胞聚集体、CD62p、CD40/M、CD40L/P、CD40L/M均明显高于对照组;无微血管病变的糖尿病组血小板-单核细胞聚集体、CD62p、CD40L/P、CD40L/M均明显高于对照组;有微血管病变的糖尿病组的血小板-单核细胞聚集体、CD62p、CD40/M、CD40L/P、CD40L/M明显高于无微血管病变的糖尿病组。血小板-单核细胞聚集体、CD62p与CD40L/P、CD40L/M有明显的正相关性;血小板-单核细胞聚集体与CD40/M呈显著正相关性。血小板-单核细胞聚集体、CD62p、CD40L/P、CD40L/M与空腹血糖有明显的正相关性。结论:血小板-单核细胞聚集体、CD62p的测定对于糖尿病微血管病变的早期诊断具有重要的临床应用和研究价值。CD40-CD40L可能通过诱导血小板的活化,血小板-单核细胞集聚参与糖尿病微血管病变。

Study of platelet-leukocyte aggregations and its correlation to CD40-CD40L in patients with diabetes mellitus

Objective:To study the difference of platelet-monoocyte aggregations in patients with diabetes mellit(DM) and its relationship with CD40,CD40L to explore the role of platelet-monoocyte and the potential clinical predicting factor in diabetic microangiopathy.Methods:Platelet-monocyte aggregations,CD62p,CD40,CD40L were detected by flow cytometry(FCM) in type 2 DM.Results:The expression rate of platelet-Monocyte aggregations,CD62p,CD40/M,CD40L/P,CD40L/M in type 2 DM group with microangiapathy was signifinantly higher than that of control group.The expression rate of platelet-monocyte aggregations,CD62p CD40L/P,CD40L/M in type 2 DM group without microangiapathy was signifinantly higher than that of control group.The expression rate of platelet-monocyte aggregations,CD62p,CD40/M,CD40L/P,CD40L/M in type 2 DM group with microangiapathy was signifinantly higher than that of 2 DM group without microangiapathy.There was a positive correlatin between platelet-monocyte aggregations,CD62p and CD40L/P,CD40L/M.There was a positive correlatin between platelet-monocyte aggregations and CD40L/M.There was a positive correlatin between platelet-Monocyte aggregations,CD62p,CD40L/P,CD40L/M and glucose.Conclusion:The measurement of platelet-monocyte aggregations,CD62p is of clinal value of apllication and study in the early diagnosis and pathophysioloy of type 2 diabetes mellitus with microangiopathy.CD40-CD40L interaction may play an important role in diabetic microangiopathy,which may induce platelet activation and platelet-monocyte aggregations.
