硬膜外自控镇痛术用于分娩镇痛的研究

目的观察硬膜外自控镇痛术用于分娩镇痛的临床效果。方法60例产妇(观察组)在产程中应用硬膜外自控镇痛术;另外60例产妇(对照组)在产程中未采取干预性镇痛措施,观察两组产妇的疼痛程度、产程、分娩方式、胎儿窘迫、新生儿Apgar评分情况。结果观察组疼痛程度明显减轻,与对照组比较差异有显著性意义(P<0·05);观察组第一产程活跃期与对照组间差异有显著性意义(P<0·01),两组分娩方式间差异有显著性意义(P<0·05)。结论硬膜外自控镇痛术能明显地减轻产妇分娩的痛苦,缩短第一产程活跃期,降低剖宫产术率,值得在产科推广使用。但其价格不菲,须视产妇的经济状况而定。

A Study and Analysis of Patient Controlled Epidural Analgesia on Analgesic Birth

Objective To investigate the effects of analgesia birth by patient controlled epidural analgesia (PCEA)during labor.Methods One hundred and twenty pregnant women were enrolled into this study from March 201 to September 2001.Sixty pregnant women applied to patient controlled epidural analgesia served as the observational group.The rest were the control group.The painful degree,stage of labor,delivery method,fetal distress,the Asgar score of the new-born infants are observed in the parturient of the two groups.Results The pain of the observational group was more relieved than that of the control group (P<0.05).The active phase of the first labor stage of the observational group was shorter than that of the control group (P<0.001).Compared with the control group,the incidence of cesarean section were lower,too (P<0.01).Conclusion The application of PCEA during labor could relieve the pain of pregnant woman.It also shorted the active phase of the first stage and lowered the incidence of cesarean section.It is worth being used widely.But it is expensive,the application need considering the economics of the pregnant woman.
