倦怠治疗对中青年男性2型糖尿病患者糖代谢控制及慢性并发症的影响

目的:探讨倦怠治疗对中青年男性2型糖尿病患者糖代谢控制及慢性并发症的影响.方法:采用国际通用的Maslach倦怠量表(MBI-GS)对284例患者进行倦怠治疗程度的调查,观察倦怠治疗对糖代谢控制及慢性并发症的影响.结果:284例中青年2型糖尿病患者倦怠治疗处于偏高水平,35.2%的患者存在一定程度的情绪耗竭,治疗依从性差,其中重度占12.7%,轻度占22.5%;23.6%的患者存在去人格化,其中重度占9.2%,轻度占14.4%;24.6%的患者缺乏工作成就感,其中重度占8.1%,轻度占16.5%;情绪衰竭和低成就感对糖代谢影响较显著,与正常组相比,轻、重度情感衰竭和低成就感患者血糖水平及GHbA1c水平显著高于正常患者(P＜0.05,P＜0.01);而去人格化程度对糖代谢影响较小,仅在重度去人格化患者中血糖水平及GHbA1c水平中有所升高(P＜0.05);情绪衰竭和低成就感对糖尿病大血管和微血管并发症影响较显著(P<0.05);去人格化程度对糖尿病大血管和微血管并发症影响不显著.结论:中青年2型糖尿病患者倦怠治疗处于偏高水平,护理人员应采取有力措施做好健康教育,提高治疗依从性,预防中青年糖尿病患者倦怠治疗状态.
