妊娠期糖尿病筛查的临床应用及意义

目的:探讨开展妊娠期糖尿病(GDM)筛查的临床应用及其意义。方法:对2000年1月~2005年1月在我院妇产科门诊2654例行产前检查者进行妊娠期糖尿病(GDM)的筛查,并对其产妇住院分娩情况进行监测及分析。结果:糖筛查阳性者278例,阳性率达10·47%,确诊为GDM58例,占2·19%,诊断为糖耐量受损(GIGT)62例,占2·34%;OGTT异常为120例(GDM+GIGT),占4·52%,OGTT异常120例中发生胎儿宫内窘迫36例、胎盘早剥18例、巨大胎儿26例、妊高征14例、产后出血10例、新生儿低血糖18例、新生儿窒息8例、早产4例、死胎6例。结论:开展孕妇糖筛查可预防糖尿病所致的产科并发症的发生,对保证母婴安全有极其重大的意义,有糖尿病家族遗传史者并发症较多,对诊断为GDM者经过控制饮食及降糖治疗可减少产科并发症的发生。

The Clinical Application and Sense of Select about GDM

Objective:To probe into the clinical application and sense of select about GDM.Methods:Select 2 654 cases who were antenatal examination in our courtyard gynaecology and obstetrics outpatient service from January 2000 to October 2004, monitoring and analysis the parturition situation of the lying-in women in our hospital.Results:There were 278 cases were masculine, the rate was 10.47% ,diagnosis 58 cases were GDM, the rate was 2.19%, diagnosis 62 cases were GIGT, the rate was 2.34%.GDM GIGT =120cases ,the rate was 4.52%, 36 cases were distress,18 cases were abruption,26 cases were fetal macrosomia ,14 cases were PIH,10 cases were postpartum hemorrhage,18 cases were neonatal hypoglycemia,8 cases were neonatal asphyxia,4 cases were premature delivery,6 cases were fetal death.Conclusion:Select about GDM can take precautions against the grave complication of obstetrical department happen, there is energetic sense to keep the female and infant's security; diabetes family heredity case history person's complication is more; to the diagnosis in the interest of GDM person through control the diet and drop blood grape sugar treats can decrease happening of obstetrical department complications.
