蝶骨嵴内侧型脑膜瘤的显微外科治疗

摘    要:目的 探讨蝶骨嵴内侧型脑膜瘤的手术治疗方法及其疗效。方法 回顾性分析2005年1月至2015年4月手术治疗的30例蝶骨嵴内侧型脑膜瘤患者临床资料,肿瘤切除采用翼点入路或改良翼点入路。结果 肿瘤全切除21例,次全切除6例,大部分切除3例。23例术后随访2个月~10年,预后优良(可以正常活动,KPS评分80~100分)16例,一般(生活可以自理,KPS评分60~<80分)4例,差(生活不能自理,KPS评分≤60分)3例;复发2例。结论 术前制定个体化的手术方案、术中精细操作和妥善处理相关血管和神经,可提高手术疗效,减少并发症。

Abstract:Objective To explore the strategy of microsurgery for the medial sphenoid ridge meningiomas. Method The clinical data of 30 patients with medial sphenoid ridge meningiomas, who underwent microsurgery through pterional or extended pterional approach from January, 2005 to April, 2015, were analyzed retrospectively. Results Of 30 patients with medical sphenoid ridge menigiomas, 21 received the total resection of the tumors, 6 subtotal and 4 greatly part. Conclusion The curative effects of microsurgery on the meningiomas may be enhanced and the postoperative complication may decreased by the individualized surgery plan made before the operation, meticulous operative manipulation and intraoperative protection of the related blood vessels and nerves, in the patients with medical sphenoid ridge meningiomas.
