美托洛尔改善扩张性心肌病心力衰竭的临床研究

目的　观察美托洛尔对扩张性心肌病(DCM)、心力衰竭的临床疗效。方法　 3 0例DCM伴心力衰竭患者在利尿、A CEI、洋地黄及硝酸盐制剂治疗的基础上 ,随机分为美托洛尔组及常规治疗组 (每组 1 5例 )。治疗前及治疗 4个月后启用超声心动图测定左室缩短分数 (FS) ,射血分数 (EF) ,左室舒张末内径 (LVDD) ,室间隔厚度 (LVS)和左室后壁厚度 (LVPW)。结果　治疗 4个月后两组心功能明显改善。美托洛尔组心功能改善较常规组更明显 ;美托洛尔组与常规组LVDD分别为61 .5± 6.5与 67.2± 6.1 ,P <0 .0 5 ,EF分别为 48.± 9.3与 40 .3±60 3 ,P <0 .0 5 ,D/T分别为 4.1± 0 .7与 4.6± 0 .5 ,P <0 .0 5。LVDD ,D/T ,LVPW美托洛尔组治疗后与治疗前以及与常规组治疗后相比改善显著 (P <0 .0 5 )。结论　美托洛尔能显著改善DCM心力衰竭心功能及逆转心肌重构。

Clinical Study of Metoprolol on the Improvement of Dilated Cardiomyopathy

Objective To observe the clinical therapeutic effect of metoprolol on dilated cardiomyopathy (DCM) and heart failure. Methods 30 patients with DCM and heart failure, who were administered with routine treatment, were divided into metoprolol group and control group in the course of 4 months' treatment. The parameters of before- and after treatment, such as left ventricular fractional shortening (FS), ejection fraction (EF), left ventricular diastolic diameter (LVDD), and thickness of ventricular septum (VS) and left ventricular posterior wall (LVPW), were measured with ultrasonic cadiogram. Results Heart function improved clearly in the two groups after 4 months' treatment. More significant improvements were observed in metoprolol group than in control group according to the comparison of some parameters between the two groups. Conclusion It is deemed that metoprolol can improve heart function of patients with DCM, and reverse the reconstruction of heart muscle.
