术前应用抗生素对急性阑尾炎术后伤口感染的影响

目的：探讨术前应用抗生素对急性阑尾炎术后伤口感染的影响。方法：收集急性单纯性阑尾炎45例（A组）,急性化脓性阑尾炎63例（B组）,急性坏疽性阑尾炎42例（C组） 以术前0～2h为限分为术前应用组和未应用组,并对两组伤口感染情况进行分析。结果：A组：术前用药29例,未用药16例,伤口感染率分别为0和6.25%,P〉0.1 B组：术前用药33例,未用药30例,伤口感染率分别为3.03%和23.33%,P〈0.025 C组：术前用药27例,未用药15例,伤口感染率分别为11.11%和40%,P〈0.05。结论：急性阑尾炎术前应用抗生素能有效防止术后伤口感染的发生,合理、正确、适时地应用是关键。

Effect of Prophylactic Aantibiotics on Preventing Wound Infection in Patients with Acute Appendicitis

Objective To study the impact of prophylactic antibiotics in the wound infection after emergency appen- dectomy of acute appendicitis. Methods Patients with acute simple appendicitis （group A）, acute pyogenous appen- dicitis （group B） and acute gangrenous appendicitis （group C） were selected for this study. Those cases were divided into application of preoperative prophylactic antibiotics group and non - application group, then the situation of wound infection was analyzed. Results In Group A： 29 patients accepted the prophylactic antibiotics, 16 cases did not, the wound infection rate was 0 and 6.25% respectively, P 〉 0.1 ;In group B： 33 patients with preventive medication, 30 cases without, the wound infection rate was 3.0% and 23.33 % respectively, P 〈 0. 025;In group C：27 patients with preventive medication, 15 cases not, the wound infection rate was 11.1% and 40% respectively, P 〈 0.05. Conclusion Prophylactic antibiotics in acute appendicitis can prevent wound infection effectively, the key point being reasonable, accurate and timely application.
