胰十二指肠切除术后胰瘘预防的体会

目的：总结壶腹周围癌胰十二指肠切除术后胰瘘预防的体会，方法：所有７０例病例均采取Ｃｈｉｌｄ’ｓ术式进行消化道重建。结果：术后共发生胰瘘１例，胰瘘发生率为１．４％。结论：胰瘘的发生与术式和操作技术有密切关系。胰腺残端和胰管断端的恰当处理，及术后充分的内外引流，是预防胰瘘发生的重要措施。

Experience on the Protection of Pancreatic Fistula after Pancreaticoduodenectomy

Objective: To summarize the experience on the protection of pancreaticfistula after pancreaticoduodenectomy in the treatment of periampullary cancer. Methods: Child 's operative technique was adopted to all of 70 consecutive patients in the reconstruction of alimentary tract. Results: Postoperative pancreatic fistula occurred in one patient, with a pancreatic fistula rate of 1. 4 %. Conclusion: There is a closer relationship between the occurrence of pancreaticfistula and the operation technique. Proper management of the remained ends of pancreas and pancreaticduct, as well as postoperative internal and external drainages could take the important roles in the prevention against pancreatic fistula.
