高危人群梅毒检测结果分析和方法学评价

目的了解本地区高危场所从业人员梅毒感染状况,评价梅毒检测方法,为制定干预措施和实验室检测策略提供依据。方法采用TRUST与ELISA方法进行检测,阳性结果均用TPPA进行确认。结果1 978份血清标本中,TRUST阳性34份,阳性率1.72%;ELISA阳性125份,阳性率6.32%,两种方法阳性率有显著性差异（χ^2=54.26,P〈0.05）,TRUST检测阳性标本ELISA检测均为阳性。125份阳性标本经TPPA确认阳性122份,ELISA与TPPA阳性结果无显著性差异（χ^2=3.04,P〉0.05）,TRUST与TPPA阳性结果有显著性差异（χ^2=132.02,P〈0.05）;各年检测结果无显著性差异（χ^2=0.80,P〉0.05）。结论ELISA可作为高危人群梅毒螺旋体感染的一种理想检测方法,对控制梅毒的性传播有重要意义。但对ELISA阳性标本应做TPPA确认。

The outcome analysis and methodology evaluation on detection of syphilis in high risk group

Objective To study the epidemic dynamics of syphilis in high-risk work-places in the Yanting county and evaluate methods of testing syphilis, so as to provide a scientific basis for adopt appropriate measures to test and control the infectious disease. Methods Two methods of TRUST and ELISA were used to test 1978 sacrum samples.The positive results were confirmed by TPPA. Results By TRUST, 34 of 1978 samples were tested positive and the rate was 1.72%. By ELISA, 125 samples were positive and the rate was 6.32%. There was different between TRUST and ELISA （X^2=54.26,P〈0.05）.All the samples which were tested positive by TRUST were also tested positive by ELISA.Among 125 positive samples by ELISA, 122 were confirmed by TPPA. There was no statistics difference between ELISA and TPPA（X^2=3.04, P〉0.05 ）.There was different between TRUST and TPPA （X^2= 132.02, P〈0.05 ）, and there was no difference between every years （X^2=0.80,P〉0.05）. Conclusion ELISA is an ideal chemical examination to test syphilis spirochaetae in high-risk people. It has very important significance in controlling the spread of syphilis. But ELISA positive samples should be confirmed by TPPA.
