三种粘蛋白在胃癌组织中表达及其临床意义

目的 研究粘蛋白（MUC1，MUC2，MUC5AC）在胃癌组织中表达及它们与临床病理学行为之间的关系。方法 应用免疫组化S－P法检测94例原发性的胃癌组织标本中MUC1，MUC2，MUC5AC在胃癌组织中的表达。结果 1.MUC1的阳性表达率为82％，不同组织学之间的表达有明显差异（P＜0.05)，在中高分化管状腺癌中表达最强，表达程度与病人年龄、淋巴结有无转移及肿瘤大小密切相关（P＜0.05)；2.MUC2表达的阳性率为84%，不同组织学类型肿瘤组织中的表达有显著性差异（P＜0.05)，在粘液腺癌中表达最强100％（7／7）；3.MUC5的阳性率为40％（38／94）,除性别之间存在差异外，与其它的临床资料无关。4.相关分析发现MUC1的表达与MUC2的表达呈正相关。结论 1、MUC1在不同的组织学类型的胃癌中表达不同，在中高分化腺癌中最高，并与淋巴结转移、肿瘤大小有关；2.MUC2在不同的组织学类型的胃癌中表达不同，粘液腺癌中表达最强，可作为研究粘液腺癌的较好指标；3.MUC5AC在胃癌组织中的表达下降，可作为研究胃癌发生、进展的较好指标。4.MUC1和MUC2之间可能有协同作用。

Significance of the expression of different kinds of mucins in gastric carcinomas

Objective To investigate the significance of the expression of different kinds of mucins in gastric cancers and the relationship between them. Methods Detected the proteins by immunohistochemical and defined the type of cancers according to the WHO criterion. Results The rate of MUC1 gene expression was 82%, its expression was related with the types of the cancers , the age of patients, the involvement of lymph nodes and the tumor sizes, but there was no significant difference between the patients' age and sex ,the location of tumor, and the metastasis to other organs. The rate of MUC2 gene expression was 84%, its expression is also related with types of cancers , and 100% mucinous carcinoma showed a strong expression ,but there was no relationship with other clinical outcomes. The rate of MUC5AC gene expression was 40%, but its expression was not related with any of the characteristics investigated at present. There was a positive relationship between MUC1 and MUC2. Conclusion MUC1,MUC2 are good markers for different types of gastric cancer. MUC1 gave a strongest expression in tubercular carcinomas, and is related to the invlvemend of lymph nodes and tumor size.MUC2 is an excellent indicator for mucinous carci-noma. MUC5AC is also a good marker in studying the progress of gastric carcinoma.
