超声龈下刮治同步药物冲洗对龈下微生物的影响

摘    要:目的 :比较Master 40 0超声龈下刮治同步药物冲洗系统与传统超声龈下刮治 +药物冲洗对龈下微生物的影响及临床疗效。方法 :选取慢性成人牙周炎患牙 2 0个 ,随机分入实验组 (超声龈下刮治同步药物冲洗 )和对照组 (传统超声龈下刮治 +药物冲洗 )。记录治疗前、后 0、7、14d的探诊出血 (BOP)、探诊深度 (PD) ,并采集龈下菌斑样本培养 ,观察比较总菌落形成单位 (CFU)和牙周可疑致病菌检出率及检出量的变化。结果 :治疗后两组PD、BOP、CFU和牙周可疑致病菌数量均显著减少 (P <0 .0 5 ) ;实验组牙龈卟啉单胞菌 (P <0 .0 1)和二氧化碳噬纤维菌 (P <0 .0 5 )明显少于对照组。结论 :Master 40 0超声龈下刮治同步药物冲洗系统比传统超声龈下刮治 +药物冲洗能更有效地减少龈下致病微生物的数量

Abstract:Objective:To assess the clinical and microbiologic effects of ultrasonic scaling with simultanous irrigation with chlorhexidine(CHX), conventional ultrasonic scaling or subgingival irrigation. Methods: 20 teeth in 7 patients with periodontitis were equally divided into test and control group. Patients in the test group were treated by irrigation with CHX during ultrasonic scaling(Master400),those in the control with irrigating after scaling(MiNi ). Clinical assessments of bleeding on probing (BOP),probing depth (PD) were made just before and 7 and 14 days after treatment. The subgingival microbiologic examination was performed before and 0,7 and 14 days after treatment. Result: Significant reductions in PD and BOP were observed in both groups 7 and 14 days after treatment(P<0.05). Microbiologic observations showed a marked decrease in total colony forming unit(CFU) and periodontal pathogens(P<0.05). There was a significant decrease in Porphyromonas gingivalis(P<0.05) and Capnocytophaga(P<0.05) in test group than in the control. Conclusion: The results indicate that both Master 400 and MiNi are effective in the treatment of periodontitis. Subgingival scaling by Master 400 combining with simultaneous CHX irrigation was more effective in reducing the subgingival microbes, especially Porphyromonas gingivalis and Capnocytophaga.
