前列腺电切术(TURP)中注射用血凝酶对凝血功能的影响

目的:观察注射用血凝酶在前列腺电切术中对PT、F IB、APTT、TT等凝血指标的影响。方法:选择拟行前列腺电切手术的病例60例,随机分为治疗组和对照组,每组各30例。两组病人手术指征及麻醉方法选择无差异。治疗组于手术开始后5分钟静脉给予注射用血凝酶2μ;对照组于手术开始后5分钟静脉给予0.9%生理盐水5m l。两组病人分别于术前24h及手术结束前5m in采静脉血测定凝血酶原时间(PT)、凝血酶时间(TT)、活化部分凝血酶时间(APTT)、纤维蛋白原(F IB)。观察两组病人术中的血压、心率、呼吸频率、动脉血氧饱和度、出血量及生命体征。结果:对照组PT、APTT随时间延长明显增加;治疗组PT、APTT无明显改变。TT、F IB在两组无显著变化。结论:注射用血凝酶在前列腺电切术中可明显减少手术过程中的出血量。

The Influence of Hemocoagulase Atrox for Injection on Blood Coagulation During Transurethral Resection of the Prostate

Objective: To investigate the effects on coagulation of hemocoagulase in Transurethral Electro-Vaporization of the Prostate operation.Methods: Sixty patients undergoing TURP operation were randomly devived into two groups: Hemocoagulase group receiving 2μ Hemocoagulase(30 patients),and control group receiving normal saline(30 patients).The effects on hemostatic time,hemorrhagic volume,hemorrhagic volume per square unit,and body coagulation time(PT,TT,APTT,FIB)were observed.Results: There was no significant difference in age,weight,baselineTT,PT,APTT and FIB between the two groups;no significant difference in perioperative homodynamic change between two groups.PT and APTT were significant prolonged in control group than those in Hemocoagulase group(P<0.05).There was no difference in FIB and TT between the two groups.Conclusion: Hemocoagulase plays a good haemostatic role in TURP operation.
