HLFO与LFO方案治疗大肠癌的对比研究

目的观察拓僖(hydroxycamptothecine,HCPT)联合亚叶酸钙(LV)、氟尿嘧啶(5-Fu)、草酸铂(L-OHP)组成的HLFO方案治疗大肠癌的近期疗效和毒性反应,并与后三种药组成的LFO方案进行比较.方法采用随机的方法将83例大肠癌患者分为HLFO方案组(A组)41例和LFO方案组(B组)42例.均每21 d为一周期,连续治疗3个周期.结果 HLFO组有效率为52.6%(10/19),LFO组45.5%(10/22),差异无显著性.两组主要毒副反应相似,均为骨髓抑制、消化道反应和神经毒性.结论 HLFO方案较LFO方案疗效有提高趋势,可通过扩大观察病例数量和增加化疗周期来进一步评价其临床价值.

Comparative study of therapeutic efficacy of HLFO and LFO in human colorectal cancer

Objective To observe the therapeutic effect and toxic effects of hydroxycamptothecine(HCPT) combined with leucovorin,5-fluorouracil(LV/5-Fu) and oxaliplatin(L-OHP) in the treatment of colorectal cancer,and to compare this treatment with the schedule of oxaliplatin combined with leucovorin and 5-fluorouracil.Methods In a randomized manner,83 patients with colorectal cancer were divided into HLFO group and LFO group.The treatment schedule was recycled every 21 days and the patients were treated for 3 cycles.Results The total response rates for the HLFO group and LFO group were 52.6%(10/19)and 45.5%(10/22) respectively,but there was no statistically significant difference between the two groups.The major toxic effects were similar.They were slight myelosupression, gastrointestinal tract reactions and neuro-sensory toxicity.Conclusion Compared with LFO,the therapeutic efficacy of HLFO tended to improve.Its value can be evaluated by enlarging the quantity of observed patients and increasing chemotherapy cycles in future.
