以唇动脉弓为蒂的唇瓣解剖学研究及临床应用

目的通过对上、下唇动脉的解剖学研究来说明以唇动脉弓为蒂的唇复合组织瓣修复唇缺损术式设计的科学性。方法对21具尸体唇部进行解剖学观测，研究唇动脉的起源、走行分布、外径及吻合情况。对42例病例进行回顾性总结，评价以唇动脉为蒂的唇复合组织瓣修复唇缺损术式的治疗效果。结果面动脉在口角处发出上唇动脉和下唇动脉，在上唇及下唇左右侧的唇动脉互相吻合形成一围绕口的冠状动脉环，位置恒定。42例唇复合组织瓣全部成活，伤口I期愈合，外形满意。结论唇动脉弓的位置恒定、变异较少，以唇动脉弓为蒂的唇复合组织瓣血供可靠，术后效果满意，是一种理想的修复口唇缺损的术式。

Anatomical Study and Clinical Application of Labial Arterial

Objective By observing labial arterial arch anatomical structure,the study was aimed to prove the compound flap pedicled with arteriae ]abiales was an ideal method of repairing the lip defect. Methods 21 adult cadavers were dissected. The origin, course, branches, diameters and vessels network of arteriae labiales were observed specially. 42 cases were reviewed and the therapeutic effect of this method was evaluated. Results Facial artery diverged superior labial artery and inferior labial artery on angle of mouth. Arteriae labiales of right and left connected on upper lip and lower lip and form a loop, which was constant. All the compound flaps survived completely without any complications. All cases obtained satisfactory results functionally and esthetically. Conclusion The compound flap pedicled with arteriae labiales was constant and the blood supply of the compound flap was reliable. It can be used to repair full lip defect with the same kind tissue,and obtain satisfactory appearance. It was an ideal method of repairing the lip defect.
