The Efficacy of Low-dose Erythromycin on Feeding Intolerance in Preterm Infants by Systematic Review

Objective:To investigate the efficacy of low-dose erythromycin 3~15 mg/(kg·d)] on feeding intolerance in preterm infants(gestational age <37 weeks) by systematic review.Methods: All randomized controlled trials on the effects of low-dose erythromycin in the treatment of feeding intolerance in preterm infants obtained by searching Cochrane library,PubMed,EMBASE,CBMdisc,CNKI were included.Meta-analysis was done by Review Manager 5.0 software.Results: Seven trials with totally 266 preterm infants were included.There were no significant differences in gestational age,weight and sex between low-dose erythromycin and control groups.Compared with control group,the preterm infants treated with oral low-dose erythromycin had shorter days of achieving full enteral feeding(weight mean difference,WMD-3.49,95%CI:-4.26~-2.71).The subgroup(gestational age ≤32 weeks) also had shorter days of achieving full enteral feeding(weight mean difference,WMD-2.95,95%CI:-3.95~-1.96).The preterm infants treated with low-dose erythromycin had shorter days of total parental nutrition(weight mean difference,WMD-2.45,95%CI:-4.49~-0.41).Conclusions: The results of this meta-analysis support that low-dose erythromycin have some therapeutic effects on feeding intolerance in preterm infants.However,better designed,double blind and randomized controlled trials with large sample size and long-term follow-up are needed for further evaluation of the efficacy and safety of low-dose erythromycin therapy.
