妊娠期胆汁淤积症对新生儿中枢神经的影响

摘    要:妊娠期肝内胆汁淤积症时在各种机制作用下母儿胆汁酸淤积 ,通过胆汁酸、胆红素的细胞毒作用 ,影响胎盘血流灌注、血液携氧能力、机体抗氧化防御系统引起胎儿窘迫、窒息致中枢神经系统缺氧损伤。神经细胞中钙稳态是保证其结构完整和功能正常的重要因素 ,烯醇化酶是缺氧缺血性脑损伤早期的标志酶。通过检测新生儿脐动脉血中的游离钙 ,烯醇化酶浓度来预测脑损伤及其严重程度 ,对早期干预具有十分重要的意义

Abstract:In intrahepatic cholestasis of pregnancy ( ICP), bile acid both in mother and fetus bodies silts up. By cell toxic action of bile acid and bilirubin ,it affects placental blood perfusion, ability of the blood to bring oxygeon, anti-oxidase defense system and cause fetal distress and asphyxia, resulting in ischemic injury of CNS. Calcium homeostasis in nerve cells is an important factor to guarantee construction integrity and normal function of the nerve cells. Enolase is a tagging enzyme of ischemic and hypoxic brain injury in earlier period. Detection of concentrations of free calcium and enolase in fetal umbilical artery can predict the degree of brain injury, so it is of great importance for early intervention of the disease.
