银屑病患者合并代谢性疾病调查分析及其胰岛素抵抗状态评估

摘    要:目的:调查银屑病患者中代谢性疾病所占比例,并评估银屑病患者胰岛素抵抗状态。方法:收集银屑病患者180例和正常人200例,比较两组伴发代谢性疾病的比例。同时在上述人群中随机选取银屑病患者70例和正常对照40例,采用葡萄糖氧化酶法及化学发光法测定血清空腹血糖和空腹胰岛素水平,并应用稳态模型法计算胰岛素抵抗指数(HOMA—IR),比较两组间水平。结果:银屑病患者伴发高血压(OR=2.39)、2型糖尿病(OR=4.27)、高血脂(OR=2.21)、肥胖(OR=3.04)及代谢综合征(OR=5.21)的比例高于对照组(P值均〈0.05),两组间心脏疾病的比例未见明显差异(OR=1.11,P〉0.05)。银屑病患者胰岛素抵抗指数高于正常对照组分别为2.54(0.62~28.71)、2.01(0.56—6.39),P〈0.05]。结论:银屑病患者伴发代谢性疾病比例高于正常人。银屑病患者存在胰岛素抵抗,且胰岛素抵抗指数高于正常人。

Abstract:Objective:To survey the prevalence of metabolic diseases in psoriasis patients, and to evaluate the condition of insulin resistance in psoriasis patients. Methods:The proportion of metabolic diseases in 180 psoriasis patients and 200 normal subjects were analyzed. Fasting blood glucose and insulin levels in 70 psoriasis patients and 40 control subjects randomly selected from the above population were determined by glucose oxidase and chemiluminescence methods, respectively. IR index was calculated using the steady state model of HOMA. Results:The proportion of hypertension( OR = 2.39 ), type 2 diabetes ( OR = 4.27 ), hyperlipidemia ( OR = 2.21 ), obesity ( OR = 3.04 ) and metabolic syndrome ( OR = 5.21 ) associated with psoriasis in patients group was statistically higher than that of normal controls (P 〈 0. 05 ). The proportion of heart dis- eases between the two groups was not statistically significant( OR = 1.11 ,P 〉 0.05 ). IR index in psoriasis group was significantly higher than that in control group 2. 54 (0. 62 - 28.71 ) vs 2. 01 (0.56 ~ ), P 〈 0. 05 ]. Conclusion: The proportion of metabolic diseases in psoriasis patients is higher than that of normal controls. Psoriasis patients have insulin resistance, and insulin resistance index is higher than that of normal controls.
