糖尿病痛性神经病变自体骨髓干细胞移植疗效观察

目的观查自体骨髓干细胞移植治疗糖尿病痛性神经病变（DPN）的临床疗效,探讨影响因素及存在问题。方法顽固性糖尿病痛性神经病变患者9例共进行12次自体骨髓干细胞移植治疗。采集骨髓,分离出干细胞,配成干细胞混悬液,进行下肢肌肉注射。结果治疗后,患者冷感、麻木、疼痛等明显好转,移植1~3个月后患者神经电生理检测、深感觉域值等改善,未出现并发症和不良反应。结论采用自体骨髓干细胞移植治疗糖尿病痛性神经病变,方法简便、安全有效,值得临床推广;但仍存在一些问题,需对该治疗的作用机理、方法、疗效评判等深入研究。

Clinical Observation on Autologous Transplantation of Bone Marrow Mononuclear Cells for Painful Diabetic Neuropathy

Objective To evaluate the clinical efficacy of autologous marrow stem cells transplantation for painful diabetic neuropathy（PDN）. Methods 9 patients with PDN were treated by autologous transplantation of bone marrow mononuclear cells for 12 times. Marrow was collected after patients were extradural anesthesia of spinal cord and stem cells in marrow were separated and injected intramuscularly into lower limbs. The clinical and laboratory findings were monitored after 1 to 3 months. Results One or three months after the stem cell transplantation, severe pain lameness.local cool-feeling, nerve conduction studies, electro- myography and quantitative sensory testing were improved significantly. No related complication or adverse effect was observed during 3 months of observation. Conclusions Transplantation of autologous marrow stem cells might be a simple, safe and effective method for the treatment of refractory PDN. But some subjects associate to methods, mechanism and effect of treatment need our further research.
