实时三维超声心动图评价法洛四联症患者手术前后右心室收缩功能

目的应用实时三维超声心动图（RT-3DE）评价法洛四联症（TOF）患儿术前及术后右心室收缩功能变化。方法应用RT-3DE测量29例TOF患儿术前及术后1周、6个月的右心室舒张末期容积（RVEDV）、右心室收缩末期容积（RVESV）、右心室每搏输出量（RVSV）及右心室射血分数（RVEF）。结果 29例TOF患儿中,12例术前RVEF低于50%（12/29,41.38%）。术后1周RVESV较术前增加,RVEF较术前减小,术后6个月RVEDV、RVESV较术前增加,RVEF值较术前减低（P均〈0.05）。随着心功能降低,三维右心室容积曲线（RVVC）逐渐低平、收缩峰值后移。结论 RT-3DE可准确、定量评价TOF患儿术前、术后右心室功能变化。

Pre- and post-operation evaluation on right ventricular systolic function in tetralogy of Fallot patients with real-time three-dimensional echocardiography

Objective To evaluate right ventricular systolic function pre-and post-operation in tetralogy of Fallot (TOF) patients with real time three dimensional echocardiography (RT-3DE). Methods Twenty-nine children with TOF underwent RT-3DE. Right ventricular end diastolic volume (RVEDV) and end systolic volume (RVESV) were outlined by the analysis software of RT-3DE, and right ventricular ejection fraction (RVEF) was calculated 1 week and 6 months after operation. Results RVEF was 50% lower in 41.38% (12/29) patients with TOF pre-operation. RVESV increased while RVEF decreased 1 week after operation. Six months afer operation, RVESV and RVESV increased while RVEF decreased compared with pre-operation. The right ventricular volume curve (RVVC) flattened and systolic peak delayed in RT-3DE with RVEF decreasing. Conclusion RT-3DE can be used as an accurate, fast method to determine right ventricular volume and function in TOF patients pre- and post-operation.
