OSCE结合情景案例在临床综合技能考核中的应用

摘    要:目的 探讨客观结构化临床考试(OSCE)考核联合情景案例在实习生临床综合技能考核中的应用效果及价值。方法 选取2014届临床医学专业实习生38名为对照组,应用传统OSCE模式,于大五实习阶段(2018年6月—2019年6月)进行临床综合技能的培训和考核;选取2015届临床医学专业实习生51名为观察组,应用OSCE结合情景案例的考核方式,于大五实习阶段(2019年6月—2020年6月)进行临床综合技能的培训和考核。对比两组医学生的考核成绩、考生满意度以及考核人员对考核模式的评价情况。结果 观察组考生在各站点得分均明显高于对照组(P <0.05);观察组考生对考核模式的满意度96.08%明显高于对照组81.58%的满意度(P<0.05);观察组考官对各站点考核模式的评价均明显高于对照组(P <0.05)。结论 OSCE结合情景案例用于临床综合技能考核中,可促进医学生规范掌握临床技能及提高满意度,获得良好考核效果。

Abstract:Objective To explore the application value and effects of objective structured clinical examination(OSCE)assessment method combined with scenario cases for medical intern students in the implementation of comprehensive clinical skills assessment.Methods 38 clinical medicine intern students in grade 2014 were assigned into control group,applying conventional OSCE assessment method,clinical skills were trained and tested in internship period in grade 5(from June 2018 to June 2019);whilst 51 clinical medicine intern students in grade 2015 were assigned into observation group,applying OSCE with scenario clinical cases,clinical skills were trained and tested in internship period in grade 5(from June 2019 to June 2020).Assessment results,satisfaction of students,acceptance of examiner were compared between observation group and control group.Results The medical students in the observation group scored significantly higher than those in the control group(P<0.05);96.08%of the examinees in the observation group were significantly higher than 81.58%in the control group(P<0.05);the evaluation of the evaluation mode by the examiner in the observation group was significantly higher than the control group(P<0.05).Conclusion The OSCE assessment combined with scenario cases can be used in clinical comprehensive skills assessment,which can promote medical students to master clinical skills in a standardized manner and improve satisfaction,and obtain good assessment results.
