PBL教学法在麻醉临床见习气管插管中的应用

摘    要:目的 探讨可视化技术辅助以问题为导向教学法(PBL)在麻醉临床见习气管插管中的应用效果。方法将本院临床见习的2016—2017级临床医学系本科学生按教学进程分为A组、B组、C组三组,A组学生采用传统教学法,B组学生采用以问题为导向的病例教学法,C组采用可视化技术辅助以问题为导向的病例教学法,记录三组的气管插管成功率、术后并发症发生率,比较各组理论考试成绩并采用问卷调查评价教学效果。结果与A组、B组比较,C组学生在总体教学效果、插管技术的掌握、基础和临床结合紧密度、学习主动性、学习有效性、综合分析能力、系统理解知识、团队协作、展现个性方面的评价优良率明显更高(P <0.05),在创新能力方面,C组学生评价的优良率虽然高于A组和B组,但组间差异无统计学意义(P> 0.05)。C组学生插管成功率高于A组及B组,组间比较差异有统计学意义(P<0.05)。A组患者气管插管术后并发症发生率为18.2%,主要表现为吞咽困难和声音嘶哑。B组术后并发症发生率是15.2%,主要表现为喉咙疼痛和声音嘶哑。C组仅有2例患者出现声音嘶哑,发生率为5.9%。C组患者术后并发症发生率虽低于A组及B...

Abstract:Objective To explore the application effect of visualization technology assisted with problem-oriented teaching method(PBL)in endotracheal intubation for anesthesia clinical trainees.Methods According to the teaching process,the undergraduate students of 2016-2017 clinical medicine department in Zhongnan Hospital were divided into three groups:A group,B group and C group.The traditional teaching method was used in Group A,the problem-oriented case teaching method was used in Group B,and the problem-oriented case teaching method was assisted by visualization technology in Group C.The success rate of endotracheal intubation and postoperative complication rate were recorded in the three groups.The theoretical test scores of each group were compared and the teaching effect was evaluated by questionnaire survey.Results Compared with group A and group B,the excellent rate of group C students in overall teaching effect,mastery of intubation technology,tightness of basic and clinical combination,learning initiative,learning effectiveness,comprehensive analysis ability,systematic understanding of knowledge,teamwork and personality is significantly higher(P<0.05).In terms of innovation ability,Although the excellent rate of students in group C was higher than that in group A and group B,there was no significant difference between groups(P>0.05).The success rate of intubation in group C was significantly higher than that in group A and group B(P<0.05).The incidence of complications after endotracheal intubation in group A was 18.2%,mainly manifested as dysphagia and hoarseness.The incidence of postoperative complications in group B was 15.2%,mainly manifested as sore throat and hoarseness.Only 2 patients in group C had hoarseness,with an incidence of 5.9%.Although the incidence of postoperative complications in group C was lower than that in group A and group B,there was no significant difference between the two groups(P>0.05).Conclusion The application of visualization assisted PBL in endotracheal intubation teaching is better than PBL teaching method and traditional teaching method.It has a high excellent rate of students'evaluation,can better improve the success rate of students'endotracheal intubation,and has a low incidence of postoperative complications.
