血管免疫母细胞性T细胞淋巴瘤临床病理分析

 目的 探讨血管免疫母细胞性T细胞淋巴瘤的诊断及鉴别诊断特点。方法 收集34例血管免疫母细胞性T细胞淋巴瘤，从病理特点、免疫组化表达及临床特征等方面进行回顾性总结分析。结果 34例血管免疫母细胞性T细胞淋巴瘤临床上均表现为不同程度的淋巴结肿大，组织学上均显示明显的血管增生、组织细胞增生，肿瘤细胞表达T细胞标记物（CD3或CD43），CD21染色显示滤泡树突状细胞增生，可与霍奇金淋巴瘤及其他类型的淋巴瘤相鉴别。结论 血管免疫母细胞性T细胞淋巴瘤临床上罕见，表现复杂多变，但肿瘤细胞通常表达T细胞标记物，组织学上注意观察有无血管增生、滤泡树突状细胞增生等，有助于其诊断和鉴别诊断。

Clinicopathologic investigation of angioimmunoblastic T cell lymphoma

Objective To investigate the pathological features and their differential diagnosis of an－gioimmunoblastic T-cell lymphoma (AITL). Methods 34 cases of AITL were collected and retrospectively studied by analyzing their pathological features, immunohistochemical expression and clinical manifestation. Results All patients present with enlarged superficial lymph node(s) in varying degrees. The histopathological and immunohistochemical analysis of the biopsy of all cases showed a obvious vascular hyperplasia and hy－perplasia of macrophage cells. Lymphoma cells expressed T-cell markers (CD3 or CD43). CD21 staining showed the hyperplasia of follicular dendritic cells. All these features are helpful in differentiation with Hodgkin lym－phoma and other types of lymphoma. Conclusion AITL is a rare clinical condition with variable presenta－tion. But the tumor cells usually expressed T-cell markers. To examine the hyperplasia of vessels and follicu－lar dendritic cells are helpful to diagnosis and differential diagnosis of AITL
