自发性脑出血脑水肿与血清铁蛋白、纤维蛋白原的相关性

摘    要:目的:研究急性自发性脑出血(SCH)后血清铁蛋白(SF)、纤维蛋白原(FIB)的含量变化与迟发性脑水肿的相关性。方法:选取SCH后迟发性脑水肿患者24例(A组)、急性SCH无迟发性脑水肿患者24例(B组)和健康查体者24例(对照组),分别进行头颅CT检查、SF和FIB检测、美国国立卫生研究院卒中量表(NIHSS)评分。结果:SCH 48 h内,A、B组SF、FIB含量增高不明显;7、14 d后,A组SF、FIB含量高于B组(P<0.05)。SCH后48 h内,SF、FIB水平和相对水肿大小之间无明显相关性(r=0.020,P>0.05),14 d后两者呈显著正相关(r=0.645,P<0.01),且A组的NIHSS评分及相对水肿体积大小明显高于B组(P<0.01)。结论:SF、FIB在脑出血后迟发性脑水肿的形成中可能起重要的作用。

Abstract:ObjectiveTo study the correlation between the changes in serum ferritin (SF) and fibrinogen (FIB) and brain edema after acute spontaneous cerebral hemorrhage (SCH). Methods: Twenty-four patients with delayed brain edema after SCH (A group), 24 patients without delayed brain edema after SCH (B group), and 24 healthy cases were employed for the study. All the subjects were given CT scan, SF and FIB examination and The National Institutes of Health Stroke Scale (NIHSS) evaluation. Results:At 48 h after SCH, the levels of SF and FIB were not obviously increased. Seven and 14 days later, the SF and FIB contents in A group were significantly increased than those in B group( P〈0.05). At 48 h after ACH, there was no correlation between SF and FIB levels as well as relative edema size ( r=0.020, P〈0.05). There was significant positive correlation between them 14 d later (R=0.645,( P〈0.01). The NIHSS score and relative edema volume in A group were significantly higher than those in B group( P〈0.01). Conclusion:SF and FIB may play an important role in delayed brain edema for-mation after SCH.
