剖宫产术同时剔除子宫肌瘤136例临床分析

目的探讨妊娠合并子宫肌瘤剖宫产术同时行肌瘤剔除的方法、安全性、可行性。方法回顾性分析我院136例剖宫产术同时行子宫肌瘤剔除,与同期150例单纯行剖宫产术的产妇对照。观察比较两组产妇术中出血量、术后2～24 h出血量、平均手术时间、术后病率、恢复肛门排气时间、切口愈合及术后住院天数。结果术后2～24 h出血量、术后病率、恢复肛门排气时间、切口愈合、术后住院天数两组比较差异无显著意义（P〉0.05）,术中出血量、手术时间肌瘤剔除组与单纯剖宫产组比较差异有显著意义（P〈0.05）。产后42 d复诊,剖宫产术同时行子宫肌瘤剔除的患者,子宫大小均恢复正常。结论剖宫产术同时行肌瘤剔除是安全、可行的,同时也避免再次手术增加患者的痛苦和经济负担。

Clinical analysis of 136 cases of cesarean combined with myomectomy

Objective To explore the safety,methods and feasibility of myomectomy during cesarean section.Methods The clinical data of 136 cases（study group） receiving myomectomy during cesarean section were retrospectively analyzed and compared with those of 150 cases of normal cesarean section.Results Between normal cesarean section group and study group,there were no significant difference in vaginal hemorrhage in 2-24 h after operation,the cotton-padded mattress disease,the time of anal air exhaust,wound healing and postoperative hospital stay（P0.05）.There was significant difference in operative blood loss,operation time（P0.05）.Further consultation in 42 days after operation of myomectomy during cesarean section showed the size of uterus got back.Conclusion Myomectomy during cesarean section is safe and feasible for appropriate cases.
