早产合并胎膜早破113例临床分析

目的探讨早产合并胎膜早破的发生因素及对母婴的影响，选择终止妊娠的方式及时机。方法对我院2008年1月-2009年1月住院分娩早产合并胎膜早破的113例孕妇的临床资料进行回顾性分析。A组妊娠满28周～33“周．B组妊娠34周．36“周，分析两组不同潜伏期、分娩方式、孕妇的新生儿结局和孕妇感染情况。结果A组呼吸窘迫综合征（RDS）和新生儿窒息、死亡率均显著高于B组（P〈0．05），不同潜伏期不同分娩方式孕妇感染情况、新生儿结局比较，无明显差异。结论对于妊娠28周～34周期间发生的胎膜早破，主张行期待疗法，力求延长孕周，减低围产儿病死率。

One Hundred and Thirteen Cases Clinical Analysis of Premature Delivery Combine Premature Rupture of Membranes

OBJECTIVE To investigate the occurrence factor and the impact on mother and child of premature delivery combine premature rupture of membranes, as well as the way and the time they choose to terminate the pregnancy. METHODS From January 2008 to January 2009, one hundred and thirteen cases clinical data of premature delivery combine premature rupture of membranes at our hospital were retrospectively analyzed. The pregnancy time length of A group was about 28 W ～ 33W plus six days, B group was about 34 W - 36W plus six days, analyzed different incubation period and mode of delivery, neonatal outcome of pregnant women and the infection condition of pregnant women. RESULTS Respiratory distress syndrome (RDS)and neonatal asphyxia, mortality rates of A group were significantly higher than the B group (P<0.05), pregnant women infection condition of different incubation period and delivery mode as well as the neonatal outcome between the two group were not significant different. CONCLUSION For premature rupture of membranes occurred between 28-34 W, we advocates symptomatic treatment to extend the gestational age, and reduceperinatal baby mortality.
