恶性肿瘤深静脉置管患者血清D-二聚体检测的临床意义

摘    要:目的研究恶性肿瘤患者深静脉置管后血浆D-二聚体的变化与血栓形成之间的关系,为恶性肿瘤患者深静脉置管后防血栓提供依据。方法使用法国STAGO全自动血凝仪对我院收治的50例恶性肿瘤患者分别在入院时和入院后30d进行D-二聚体检测,其中25例行深静脉置管,25例未行深静脉置管,比较入院时和入院30d血浆中D-二聚体水平。结果50例恶性肿瘤患者中有39例D-二聚体检测值高于正常参考值范围,两组D-二聚体检测值偏高差异无统计学意义(χ^2=0.117,P=0.733);入院后30d行深静脉置管组D-二聚体检测值偏高人数高于未行深静脉置管且(χ^2=6.818,P=0.009)。结论恶性肿瘤行深静脉置管患者有较大的血栓风险,动态监测D-二聚体有助于预防恶性肿瘤深静脉置管患者血栓形成。

Abstract:Objective To study the relationship between plasma D-dimer of malignant tumor with deep venous catheter and thrombosis,and to provide the basis for prevention of thrombosis. Methods France STAGO automatic coagulation analyzer was used to test the D-dimer of 50 patients with malignant tumor, in which 25 cases of patients had deep vein catheterization and 25 cases of patients without deep vein catheterization; The D-dimers were compared between two groups after hospital admission for 30 days. Results D-dimer values of 39 cases in 50 patients with malignant tumor were over the normal reference range and high values of D-dimer testing had no significant difference between two groups (χ^2=0.117,P=-0.733);After admission for 30 days,the D-dimer values of deep venous catheter group was higher than that of without deep venous catheter group (χ^2=6.818,P=0.009) Conclusion It is risky for malignant tumor patient to have deep vein intubation,which might induce thrombus, and dynamic monitoring of D-dimer is helpful for preventing thrombus formation among malignant tumor patients.
