未足月胎膜早破临床特点及防治研究

目的观察未足月胎膜早破临床特点及防治研究。方法回顾性分析2002年6月-2009年6月我院诊治的154例pPROM患者的临床资料。结果入院24h内终止妊娠共42例。实施期待治疗者28～33^＋6周组期待治疗时间最长；25～27^＋6周组并发症发生率最高；发病越早，围生儿RDS发生率、死亡率越高。结论pPROM的临床比较常与职业和流产、引产史及感染等有关，结合发病时间实施个体化治疗，孕周〈28周胎膜早破者不宜继续妊娠，以引产为宜；孕期已达34周或以上者原则上应尽快终止妊娠；对孕龄28～33^＋6周胎膜早破患者，可选择期待治疗；加强健康教育、及时治疗阴道感染、避免腹压突然增加、多进食富含维生素E、C的食物，对预防胎膜早破的发生有益。

Clinical Characteristics and Control of Preterm Premature Rupture of the Membranes

Objective To observe the clinical characteristics of preterm premature rupture of membranes and prevention research. Methods We analyzed retrospectively 154 cases of clinical data of patients from June 2002 to June 2009 in hospital for treatment. Results Hospitalized within 24h were 42 eases of termination of pregnancy. Looking forward to the implementation of the treatment group of 28 -33^＋6 weeks looking forward to treating the longest time;25-27^＋6 weeks group the highest rate of complications;disease sooner RDS incidence of perinatal mortality rate higher. Conclusion Clinical pPROM more frequently,and occupation,the history of abortion,abortion,infection and other relevant,combined with the implementation of individual treatment time of onset,gestational age 〈 28 weeks premature rupture of membranes are advised not to continue pregnancy to induced abortion is appropriate; pregnancy reached 34 weeks or should in principle be over as soon as possible termination of pregnancy;to 28 -33^＋6 weeks gestational age in patients with premature rupture of membranes may choose look forward to treatment;enhance health education and timely co-infection, to avoid a sudden increase in abdominal pressure, eat more rich in vitamin E,C,food,beneficial to the prevention of premature rupture of membranes occurred.
