标准外伤骨瓣开颅减压术治疗重度颅脑损伤并外伤性脑梗死的临床研究

目的探讨标准外伤骨瓣开颅减压术治疗重度颅脑损伤并外伤性脑梗死的临床效果。方法60例重度颅脑损伤并外伤性脑梗死患者随机分为治疗组和对照组，治疗组给予标准外伤骨瓣开颅减压术治疗，对照组给予常规骨瓣开颅术治疗，随访6个月评价临床疗效。结果治疗组有效率为56．7％，显著高于对照组（16．7％），两组比较有显著性差异（P〈0．05）；治疗组重残、植物状态和死亡发生率均显著低于对照组，两组比较有显著性差异（P〈0．05）。结论标准外伤大骨瓣开颅术可提高重度颅脑损伤并外伤性脑梗死患者的治疗效果，降低死亡率。

Effects of Standard Large Trauma Craniotomy in Treatment of Severe Traumatic Head Injury Complicated with Traumatic Cerebral Infarction: A Clinical Study

Objective To investigate the clinical effects of standard large trauma craniotomy in severe traumatic head injury complicated with traumatic cerebral infaretion（TCI）. Methods Sixty cases of patients with severe traumatic head injury complicated with TCI were randomly divided into treatment group and control group,the treatment group was given the surgery of standard large trauma craniotomy and the control group was given the surgery of routine craniotomy,follow up 6 months to evaluate the clinical efficacy. Results The clinical efficacy rate of the treatment group was 56.7% and the control group was 16.7% with there was a significant difference between two groups （P〈0.05）;The incidence rate of severe disability,vegetative state and death were significantly lower than those of the control group,the differences between two groups were significant （P〈0.05）. Conclusion The standard large trauma craniotomy can improve the clinical effects and reduce mortality for treating the patients with severe traumatic head injury complicated with TCI.
