厄贝沙坦联合胺碘酮治疗原发性高血压合并阵发性心房颤动的临床观察

目的评价厄贝沙坦与小剂量胺碘酮联合治疗原发性高血压合并的阵发性心房颤动（房颤）的疗效。方法将100例原发性高血压合并阵发性房颤患者随机分为胺碘酮组（对照组）、胺碘酮联合厄贝沙坦组（治疗组），治疗随访时间1年，比效两组治疗后的窦性心律维持率和对左心房功能的影响。结果治疗后第6个月，对照组和治疗组的窦性心律维持率无统计学差异（P〉0．05）；治疗12个月后，治疗组和对照组的窦性心律维持率分别为78．88％、47．65％，有统计学差异（P〈0．05）；治疗后测得他们的超声心动图治疗组患者LAD、LAEDV及LAESV较治疗前有明显下降（P〈0．05）；而对照组则无明显改善。两组治疗后的LAEF均明显增加，但治疗组增加有统计学差异（P〈0．01）。结论胺碘酮和厄贝沙坦联合治疗原发性高血压合并阵发性房颤，维持窦性心律的疗效及改善左心房功能优于单用胺碘酮。

Combination of Irbesartan and Amiodaronen in Treatmrnt of Essential Hypertension Complicated with Paroxysmal Atrial Fibrillation: An Effect Observation

Objective To evaluate curative effect of combination of Irbesartan and Amiodaronen in treatmrnt of essential hypertension complicated with paroxysmal atrial fibrillation. Methods One hundred essential hypertensive patients complicated with paroxysmal AF were randomly divided into two groups ：control group and treatment group. Irbesartan and Amiodarone were administrated to patients in treatment group while Amiodarone to patients in control group;The treatment lasted for 1 year in both groups and then the clinical cu- rative effects of the 2 groups were compared. LAD,LAEDV and LAESV were measured in two groups before and after treatment by Doppler ultrasound. Results After 6 months sinus rhythm maintenance rate in control group was similar with that in treatment groupv （P〉0.05）. After 12 months sinus rhythm maintenance rate in treatment group （78.88%） was significantly higher than in control group （47.65%）（P〈0.05）. LAD,LAEDV and LAESV in treatment group decreased significantly compared with that before treatment （P〈 0.05）. In control group,there was no significant difference. Conclusion Irbesartan combined with Amiodarone can obviously decrease the recurrence of atrial fibrillation and improve the function of left atrium in treating hypertension complicated with paroxysmal atrial fibrillation.
