岩斜区脑膜瘤的显微外科治疗(附60例报告)

目的 通过对６０例岩斜区脑膜瘤显微外科治疗的分析，探讨岩斜区脑膜瘤的显微手术治疗方法。方法 总结６０例岩斜区脑膜瘤的临床表现、神经影像学特征及显微手术方法和术后处理。结果 肿瘤全切除４３例，次全切除１０例。手术并发症率为５１．７％，死亡２例（３．３％），均死于术后脑内血肿。结论 岩斜区脑膜瘤采用经岩骨乙状窦前入路手术全切除率明显高于其它入路，显微外科技术及熟练掌握颅底显微解剖，使手术死亡率及并发症

Microsurgcal treatment of petroclival meningiomas

Objective To discusis microsurgcal treatment of petroclival meningiomas through analysis of 60 patients.Methods Clinical and neuroimaging features, operative approaches and postoperative management.Results Total resection was achieved in 43 cases, and subtotal resection in 10 cases. Rate of postoperative complications is 51 7%, two patients (3 3%) died due to postoperative hematoma. Conclusions Petroclival meningiomas can be resected by transpetrosal presgimoid approach. The rate of total resection is higher in presigmoid approach . The rate of total resection is higher in precigmoid approach than in others. Rates of postoperative mortality and complications are more reduced after using microsurgical technique and understanding mcranatomy of skull base. Outcomes are satisfied in most of total section cases.
