改良B-Lynch缝合术治疗剖宫产宫缩乏力性产后出血的临床观察

目的探讨改良B—Lynch缝合术治疗剖官产宫缩乏力性产后出血的疗效和安全性。方法回顾性分析25例剖宫产术中发生宫缩乏力大出血的患者，其中研究组11例，采用改良式B—Lynch缝合术止血，对照组14例，采用传统方法止血，观察并比较两组手术时间、产后出血量、成功率、产褥病率、血红蛋白变化，以及术后月经来潮时间。结果观察组手术时间显著低于对照组，差异具有统计学意义（P〈0．01）；产后出血量显著低于对照组，差异具有统计学意义（P〈0．05）；治疗成功率显著高于对照组，术后产褥病率明显低于对照组，差异具有统计学意义（P〈0．05）；两组患者在术后月经来潮时间差异无统计学意义（P〉0．05）。结论改良B—Lynch缝合术治疗剖宫产宫缩乏力性产后出血疗效明显，且操作简单安全，适合基层医院使用推广。

Clinical effect of modified B-Lynch suture in treatment of uterine atony hemorrhage during cesarean section

Objective To investigate the effect and security of modified B-Lynch suture in treatment of uterine atony hemorrhage during cesarean section. Methods 25 patients in uterine atony hemorrhage during cesarean section were enrolled for retrospective analysis.11 patients using the modified B-Lynch suture hemostasis were selected as observation group,and 14 cases in control group using traditional stop bleeding methods.Operative time,postpartum hemorrhage,the success rate,puerperal morbidity,hemoglobin changes,and menstruation time were observed and compared. Results The operative time and postpartum hemorrhage in observation group were significantly lower than that in the control group,the difference was statistically significant（P 〈 0.01）.The success rate in observation group was significantly higher than the control group,while postoperative puerperal morbidity was significantly lower,and the difference also has statistical significant （P 〈 0.05）.And there was no significant difference between two groups in menstruation time（P 〉 0.05）. Conclusion It is effect of modified B-Lynch suture in treatment of uterine atony hemorrhage during cesarean section.More importantly,it is simple,safe and suitable for primary hospitals.
