子宫动脉栓塞在剖宫产瘢痕妊娠中应用

摘    要:目的探讨子宫动脉栓塞术在剖宫产瘢痕妊娠治疗中的治疗效果和应用价值。方法2010年5月~2012年5月间收治50例剖宫产瘢痕妊娠患者随机分为栓塞组(30例)和非栓塞组(20例)。栓塞组在行子宫栓塞术之后再进行清宫术或病灶切除术,而非栓塞组则直接进行清官术或病灶切除术。结果栓塞组手术过程出血量为(166.7±13.5)mL显著低于非栓塞组的(866.9±33.8)mL(P〈0.01);栓塞组平均住院时间(14.5±2.0)d。β—HCG降低至正常的平均时间(18.5±2.5)d,均显著低于非栓塞组的(24.5±3.5)d和(28.0±4.0)d(P〈0.05)。结论子宫动脉栓塞术对剖宫产瘢痕妊娠治疗具有一定的应用价值,并且具有可保留子宫、微创、快速有效等优点,可以最大程度的保留患者的生育功能,是剖官产术后瘢痕妊娠有效的治疗方法。

Abstract:Objective To investigate clinical application of uterine artery embolization in cesarean scar pregnancy. Methods 50 patients with cesarean scar pregnancy in our hospital from May 2010 to May 2012 were divided into embolization group(30),treated by uterine curettage or surgical excision after uterine artery embolization and non- embolization group(20),treated by uterine curettage or surgical excision. Results After treatment,the amount of bleeding of embolization group(166.7 ± 13.5)mL was significantly lower than that of non- embolization group ( 866.9 ± 33.8)mL(P 〈 0.01).The hospitalization time and the normal time of β -HCG of embolization group(14.5±2.0)d, (18.5 ± 2.5)d were significantly lower than that of non- embolization group (24.5 ± 3.5)d,(28.0 ± 4.0)d(P 〈 0.05 ). Conclusion Uterine artery embolization for the treatment of cesarean scar pregnancy has a certain value,retain the uterus,minimally invasive,be fast and effective,can maximize the retention of patients with reproductive function and be effective cesarean scar pregnancy treatments.
