中耳乳突再根治术治疗不干耳的临床观察

目的分析乳突再根治术在不干耳治疗中的临床应用情况。方法选取2002年12月-2012年12月收治的57例行乳突再根治术的不干耳患者，术前均全面检查，并严格执行手术，分析临床手术效果，以及术中所发现的不干耳发生原因。结果手术成功率达98．2％；不干耳发生原因主要有病灶残留、引流受阻、面神经嵴偏高、面神经裸露以及复发胆脂瘤等，其中，外耳道狭窄、面神经嵴偏高及复发胆脂瘤的患者比率相对更高（P〈0．05）。结论应加强对中耳乳突根治术后出现不干耳原因的分析，并对行乳突再根治术的不干耳患者严格执行手术，以保证良好的手术效果。

Clinical observation of radical middle ear mastoid reoperation in the treatment of not dry ear

Objective To analyze the clinical condition of radical mastoid reoperation in the treatment of not dry ear. Methods Fifty-seven patients with not dry ear underwent radical mastoid reoperation during December 2002 to December 2012 were selected. The patients were done preoperative complete inspection and were strictly implemented the operation. The clinical surgical effect and causes of not dry ear that were found during operative procedure were analyzed. Results The surgery successful rate was 98.2%, the main causes of not dry ear were remains of lesion, obstructed drainage, high facial ridge, exposed facial nerve and recurrent cholesteatoma and so on, of which the proportion of patients with stricture of external auditory canal, high facial ridge and recurrent eholesteatoma were relatively higher（P ~ 0.05）. Conclusion reoperation should be dry ear to ensure good strengthened, and the surgical effect.
