腹腔镜与传统开腹胆囊切除术的临床疗效比较

目的比较腹腔镜与传统开腹胆囊切除术的临床疗效。方法选取我院从2012年2月～2013年2月所收治的60例行胆囊切除术患者作为临床研究对象，将患者分为腹腔镜组和开腹组两组，开腹组患者采用传统开腹胆囊切除术进行治疗，而腹腔镜组则采用腹腔镜胆囊切除术进行治疗。比较两组患者的手术时间、术后排气时间、住院时间、止痛药使用率、住院费用和并发症发生率。结果两组患者的手术时间（73．8±11．2）和（96．2±10．6）min、术后排气时间（22．5±6．1）和（37．8±16．8）h、住院时间（5．8±1．2）和（15．9±8．3）d、止痛药使用率20．O％和75．O％、住院费用（4678．5±513．2）和（5671．8±522．6）元、并发症发生率2．5％和15．0％的差异均有统计学意义（P〈0．05）。除住院费用一项是腹腔镜组高于开腹组外，腹腔镜组的其他指标均优于对照组。结论腹腔镜胆囊切除术具有创伤小、手术时间短、患者恢复快、并发症发生率低等优点，可在临床中推广使用。

The comparison of clinical effects laparoscope and traditional open cholecystectomy

Objective To compare the clinical effects laparoscope and traditional open cholecystectomy. Methods 60 patients who had been treated in our hospital from February 2012 to February 2013 for cholecystectomy were selected as research object. They were divided into laparoscope group and laparotomy group. Traditional open cholecystectomy was given to laparotomy group, while laparoscopic cholecystectomy was given to laparoscope group. We compared operation time, postoperative exhaust time, hospital stays, usage rate of paregoric, hospitalization expenses, and complication probability of two groups. Results The variations in operation time （73.8 ±11.2） and （ 96.2 ± 10.6 ）min, postoperative exhaust time （22.5 ±6.1） and （37.8 ± 16.8）h, hospital stays （5.8 ± 1.2） and （15.9 ± 8.3） d, usage rate of paregoric 20.0% and 75.0%, hospitalization expenses （4678.5 ± 513.2） and （5671.8 ±522.6） yuan, and complication probability 2.5% and 15.0% had statistical signifieance（P 〈 0.05）. Hospitalization expenses in laparoscope group was higher than laparotomy group. Except that, laparoscope group was superior to laparotomy group in all aspects.Conclusion Laparoscopic cholecystectomy has the advantages of little trauma, shorter operation time, fast recovery after operation, and low rate of complication. It is worthy of application and promotion in clinic.
