结直肠肿瘤合并糖尿病的围手术期处理

目的探讨结直肠癌合并糖尿病围手术期的处理原则。方法回顾性分析经手术治疗的78例合并2型糖尿病结直肠癌患者临床资料。结果术后并发症发生率：酮症酸中毒为2．56％（2／78），切口感染为7．69％（6／78），呼吸道感染为11．54％（9／78），泌尿系感染为6．41％（5／78）例，腹腔脓肿继发败血症为2．56％（2／78），吻合口瘘为3．85％（3／78），无死亡病例。结论合理运用胰岛素，严格控制血糖，对于结直肠癌合并糖尿病患者安全渡过围手术期至关重要。

Perioperative management of colorectal cancer complicated with diabetes mellitus

Objective To investigate the perioperative treatment principles of colorectal cancer complicated with diabetes mellitus. Methods Clinical data of 78 colorectal cancer patients with type 2 diabetes mellitus who were treated with surgical operation were analyzed retrospectively. Results The postoperative complications included 2.56%（2/78） ketosis acidosis,7.69% （6/78）incision infection, l l.54% （9/78） respiratory infection,6.41% （5/78） urinary tract infection,2.56% （2/78） sepsis secondary to abdominal abscess and 3.85% （3/78） anastomotic leakage,and no death occurred. Conclusion Reasonable use of insulin and strict control of blood glucose is very important for the colorectal cancer patients with diabetes mellitus to get through the perioperative period safely.
