USPIO增强MR黑血及白血成像序列检测兔动脉粥样硬化斑块的对比研究

摘    要:目的:探讨超小顺磁性氧化铁(USPIO)增强MR黑血及白血成像序列检测兔动脉粥样硬化(AS)斑块的敏感性及价值。方法:健康雄性新西兰大白兔30只,随机分为实验组20只,对照组10只。实验组采用球囊导管损伤腹主动脉内膜结合高脂饮食的方法建立兔动脉粥样硬化模型,对照组不作干预。MRI检查包括平扫、USPIO增强MRI黑血序列和白血序列。比较两种成像方法显示斑块形态、数目和成份的差异,与病理结果行对照研究,并做统计学分析。结果:12例兔AS模型成功建立,USPIO增强黑血序列T2WI及T2*WI扫描斑块中央信号减低,USPIO增强白血序列表现为管壁点状充盈缺损。扫描范围内普鲁士蓝染色阳性斑块共67个,MR黑血与白血序列对阳性斑块的检出率无显著性差异。黑血成像序列对于区分AS斑块成分较白血序列好。结论:USPIO颗粒可以被兔AS模型斑块中的巨噬细胞特异性吞噬并引起MRI信号的改变。USPIO增强MR黑血成像序列能敏感检出并反映兔AS斑块成分,有望用于对AS病变诊断及斑块稳定性的评估。
