摘    要:目的:分析耐多药铜绿假单孢菌感染对极重度慢性阻塞性肺疾病(COPD)急性加重期患者预后的影响。方法:回顾性分析50例铜绿假单孢菌感染极重度COPD急性加重期患者的临床资料,按其痰或支气管—肺泡灌洗液细菌培养结果分为耐多药菌组及非耐多药菌组,对比分析两组间的住院14d内出院例数、机械通气使用例数、病死例数。结果:两组间住院14d内出院率、机械通气使用率和病死率差异均有统计学意义,P〈0.05。结论:极重度COPD急性加重期患者发生耐多药铜绿假单孢菌感染后住院时间延长,需要应用机械通气的机率升高,且病死率升高,预后差。

Abstract:Objective:To analyze the influence of acute exacerbations of extreme-severe COPD by multidrug resistant Pseudomonas areuginosa. Methods: Analyzing retrospectively 50 extreme-severe COPD patient with muhidrug resistant Pseudomonas areuginosa. According to the sputum culture and bronchoalvolar lavage fluid, there were bacteria of multi-resistant drug group and bacteria of non-multi-resistant drug group. The differences of cases discharge after 14d hospitalization time,mechanical ventilation cases and mortality were compared groups). Result: All above parameters had significant difference between the two groups. Conclusion: Extreme-severe COPD with multidrug resistant Pseudomonas areuginosa resulted in prolonged time of hospitalization, more chance of mechanical ventilations. the mortality of these cases was significantly higher and the clinical prognosis was poor.
