低位腹主动脉临时阻断技术在骶骨肿瘤手术中的应用(附23例分析)

摘    要:目的 探讨低位腹主动脉临时阻断技术在骶骨肿瘤手术中对出血量的影响.方法 本组共23例骶骨肿瘤,其中骶骨脊索瘤12例,骨巨细胞瘤6例,神经纤维瘤3例,神经鞘瘤2例.常规使用低位腹主动脉临时阻断技术,检测血压、心率变化并记录出血、输血量及手术时间.结果 23例肿瘤病灶均完整切除,术中、术后均有不同程度输血,术后1例伤口愈合不良,2例出现部分皮瓣坏死.结论 低位腹主动脉临时阻断技术可明显减少术中出血,保证肿瘤病灶切除的彻底性和安全性,且对全身及双下肢血循环不构成损害,值得临床推广.

Abstract:Objective To investigate the effect of intermittent block of lower abdominal aorta intra-operation of sacrum huge neoplasms on volume of blood loss.Methods There were 23 cases with sacrum huge neoplasm including 12 chordoma,six giant cell tumor of bone,four neurofibroma and three neurinoma.The technique of intermittent of lower abdominal aorta was routinely intra-operation.Some parameters were recorded including blood pressure,heart rate,the volume of blood loss and transfusion,and operation time.Results All tumors were integratly excised and operations were successfully completed.Different volume of blood were transfused to every patient intra-operation and postoperation.There were one case with wound unhealing and two necrosis of the incision margin of all patients.Conclusion The technique of intermittent of lower abdominal aorta could significantly reduce blood loss intra-operation and ensure to completely excise huge neoplasma.This technique will deserve spread in clinical.
