长骨骨化性纤维瘤的X线与CT诊断

摘    要:目的:回顾分析长骨骨化性纤维瘤的X线与CT表现,探讨X线与CT诊断价值。方法:14例经手术病理证实的长骨骨化性纤维瘤,所有病例均行常规X线检查,其中6例同时采用常规CT平扫,层厚、层距5mm。结合文献,综合分析。结果:病变位于胫骨11例,股骨3例,起自骨干皮质12例.中心者2例。主要X线与CT表现为溶骨性骨质破坏.偏心膨胀性生长,粗细不均的骨性间隔,边缘增生硬化及骨化。根据其表现可分为囊状型8例、混合型4例和骨化型2例。结论:X线检查对长骨骨化性纤维瘤的诊断及鉴别诊断是有价值的。CT较X线平片能更好地显示病变结构及邻近软组织改变。结合CT检查,能提高诊断准确率。

Abstract:Objective: To retrospectively study the X- ray and CT appearance of ossifying fibroma of the long bone and to explore the diag nostic value of X-ray and CT. Methods: 14 cases of ossifying fib roma of the long bone proved by surgery and histopathology were examined by plai n film, 6 of them were scaned with unenhanced CT with 5 mm thickness and spacing respectively. These data were analyzed in combination with literature reviewing . Results: The lesion occurred in tibia in 11 cases and in femur in 3 cases. 12 lesions were origirated in the cortex of diaphysis and 2 were i n the center. The main appearances of the X-ray and CT were osteolytic bony des truction, eccentric and expansive growth uneven bony septations, and rim osteos clerosis and ossification. According to the features, 14 cases were divided into 3 types: (1) cystic (n=8), (2) mixed (n=4) and (3) ossifying(n=2). Conclusion: The diagnosis and differential diagnosis can be made correctly with X-ray manifestations in some of ossifying fibroma of the long bo ne. CT is superior to plain radiography in displaying the structure of the disea sed bone and its neighboring soft-tissue. Combining with CT examination, the di agnostic accuracy of ossifying fibroma of the long bone can be improved.
