残胃癌外科治疗临床分析

目的探讨残胃癌发生的原因及诊断治疗的方法。方法回顾性分析1998年~2006年20例残胃癌病人的临床资料。结果局部复发12例(包括吻合口和残胃),腹腔、盆腔种植复发3例,肝脏转移2例,卵巢转移2例。非手术治疗4例,手术16例;根治性切除11例,姑息性切除5例。根治性切除、非根治性切除和非手术治疗病人的中位生存时间分别为26个月、11个月和6个月。结论残胃癌的预防重在术中无瘤操作,定期随访是早期诊断的关键,积极再手术能延长生存期,提高生活质量。

Clinically analyzing the treatment of gastric remnant carcinoma

Objective To study the causes of gastric remnant carcinoma,diagnosis and treatment for gastric remnant carcinoma.Methods Twenty cases suffering from gastric remnant carcinoma between 1998 and 2006 were analyzed retrospectively.Results Local recurrence in 12 cases,abdominal and pelvic cavity transplant recurrence in two cases,two cases had liver metastasis,two cases had liver ovaries.Non surgical treatment was used in four cases,and surgical treatment was used in 16 cases,11cases were treated with radical resection and five cases received palliative resection.The Medium survival time of radical resection and non-radical resection and non-surgical treatment was 26 months,11 months and 6 months respectively.Conclusion No-touch technique at surgery play an important role in preventing gastric remnant carcinoma.Regular follow-up is the key to early diagnosis.The recurrence of gastric remnant carcinoma still needs active surgical treatment in order to prolong the survival time and improve quality of life.
